Pregnant? Congratulations! To see the midwife you will need to complete a Antenatal booking form and then the midwife will arrange your first appointment at around 8 weeks of pregnancy.
The midwife visits the surgery most Mondays so your appointments with the midwife will be on a Monday.
Antenatal booking form – Complete and return at least 2 weeks before your 1st midwife appointment

We advise all women to take folic acid 400mcg during early pregnancy and vitamin D supplements.
If you have any worries or concerns about pregnancy you can always speak to the GP before your midwife appointment.
