Med3 Fit Notes (sick note)

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

If you have been seen about your illness that is causing you to be off work already, or you need a extension of a sick note please click here or complete the form below. Otherwise you will need to book an appointment to discuss the reasons for needing a sick note.
